Insurance Information
Light of the Rockies Insurance Policy
We currently have therapists in network with Anthem BCBS, Aetna, and Victim’s Compensation.
If your counselor is an in-network provider, please provide your insurance information to Light of the Rockies using the form below. Light of the Rockies will collect the contracted rate set by your insurance company. If your insurance requires a referral or authorization, please have your primary care provider fax it to Light of the Rockies at (970) 224-4893 before your session.
If your counselor is an out-of-network provider, you will be charged the standard fee of $160 and you may request a Superbill. Please use the instructions below to submit the claim to your insurance company.

Instructions for Out-of-Network Insurance
Note: Clients using the Sliding Fee Scale are not eligible for Out of Network Billing Statements
Step 1: Verify your out-of-network insurance eligibility and benefits by contacting your insurance company.
Step 2: Receive a Superbill from Light of the Rockies.
Step 3: Submit a claim with the Superbill and proof of payment attached to your insurance company.
(It is important to retain copies for your own records.)
Step 4: Receive an Explanation of Benefits (EOB) explaining charges applied to your deductible or payments for covered services.
